SCREENING AND INTERVENTION.

The Screening and Intervention Action Team (SIAT) continues to carry out its mission of working with participating patient-centered medical homes/primary care practices to engage in a quality improvement initiative to expand and/or establish suicide screening, office protocols, referral networks, mental health partnerships and Suicide-Safe Care Pathways that are rooted in research and best practice.  This work aims to address the pressing problem of increasing rates of death by suicide in Vermont over the past two decades.
